Nurturly is seeking an exceptional Navigator to join our growing crew. Navigators guide clients and families through their care journey while supporting the Anchors who provide trusted day-to-day care and assistance. We are looking for a skilled, compassionate, and highly organized LVN who believes that dignity, connection, and peace of mind are just as important as safe, reliable care.

About the Navigator Role
The Navigator oversees day-to-day care operations and helps ensure the consistent delivery of safe, compassionate, hospitality-first services. This role connects clients, families, Anchors, and Nurturly leadership through client onboarding, service coordination, Anchor oversight, quality assurance, and regulatory compliance.
Key ResponsibilitiesHospitality & Client Experience
- Create a welcoming, reassuring, and professional experience for clients and families.
- Conduct client intake and onboarding and support a smooth transition into Nurturly services.
- Learn clients’ preferences, routines, support needs, and comfort priorities.
- Build trusted relationships and maintain proactive, respectful communication.
- Demonstrate and reinforce Nurturly hospitality standards in every interaction.
Care Operations & Service Coordination
- Coordinate day-to-day care operations to support safe, reliable, and consistent service delivery.
- Review service plans and documentation for accuracy, completeness, and alignment with client needs.
- Support thoughtful matching of Anchors based on client needs, preferences, and service requirements.
- Monitor service delivery and respond promptly to changes, gaps, concerns, or missed visits.
- Coordinate with clients, families, Anchors, referral sources, and authorized care partners as appropriate.
Anchor Leadership & Support
- Provide day-to-day oversight, guidance, and support to Anchors.
- Reinforce Nurturly standards for care, hospitality, professionalism, documentation, and communication.
- Support Anchor onboarding, orientation, training, and ongoing development.
- Coach Anchors through service concerns, performance expectations, and client-related challenges.
- Promote accountability and a supportive, high-performing team environment.
Safety, Quality & Compliance
- Respond to and investigate incidents, complaints, and service concerns.
- Identify potential safety or service concerns and coordinate timely follow-up.
- Conduct chart audits and quality assurance reviews.
- Monitor employee licenses, screenings, training, and compliance requirements.
- Maintain accurate and timely client, employee, and operational documentation.
- Support compliance with applicable Texas HHSC HCSSA requirements, Nurturly’s policies, and regulatory reviews.
Qualifications
- Current Texas Licensed Vocational Nurse (LVN) license in good standing.
- Minimum 2 years of healthcare experience.
- Strong interpersonal and communication skills.
- Demonstrates emotional intelligence, sound judgment, and professionalism.
- Highly organized and able to manage multiple priorities.
- Able to coach, guide, and hold team members accountable.
- Strong understanding of documentation, compliance, and quality assurance processes.
- Previous supervisory, team leadership, or operations experience preferred.
- Experience in home care, personal assistance services, home health, hospice, senior care, or healthcare operations preferred.
- Proficiency with Microsoft Office and electronic health record or care management systems.
- Valid driver’s license and reliable transportation.
- Must complete required training and screenings.
