The HH PTA will provide specialized physical therapy services primarily in patients' homes. This role focuses on delivering personalized rehabilitation care aimed at improving mobility, ensuring patient safety, and enhancing overall quality of life. The therapist works closely with the supervising physical therapist in addition to the Physical Therapy Manager and a multidisciplinary team to ensure effective treatment outcomes.
Responsibilities
- Conduct home visits to provide rehabilitative services
- Educate patients and families on exercises
- Document clinical findings and patient progress accurately
- Coordinate care with interdisciplinary team members
- Monitor patient progress and adjust interventions accordingly
- Maintain communication with physical therapists and clinical managers
- Ensure patient safety during all interventions
Preferred Qualifications
- Associate Degree in Physical Therapist Assistant
- Licensed Physical Therapist Assistant
- Home Health experience preferred but not required, willing to train clinicians new to home health!
- Wellsky/Kinnser experience preferred but not required
Pay: $50.00 - $60.00 per hour
