Cridersville Healthcare Center in Cridersville, OH Cis seeking a PRN Physical Therapist (PT) to join our growing In-House therapy team. Essential Duties & Responsibilities:
- Evaluate residents’ physical conditions and develop individualized treatment plans based on physician orders and clinical needs.
- Provide therapeutic interventions focused on improving strength, mobility, gait, balance, endurance, and pain management.
- Establish realistic functional goals and modify treatment plans as needed based on progress.
- Complete accurate and timely documentation in compliance with Medicare, Medicaid, and facility guidelines.
- Collaborate with physicians, nursing staff, occupational therapists, speech therapists, and families regarding resident progress and discharge planning.
- Educate residents, families, and caregivers on treatment plans, exercises, and safety precautions.
- Participate in care plan meetings and interdisciplinary team discussions.
- Ensure compliance with all state, federal, and company policies, including infection control and safety standards.
- Supervise and direct Physical Therapist Assistants (PTAs) and rehabilitation aides as appropriate.
- Support facility initiatives related to quality measures, fall prevention, and restorative nursing programs.
Qualifications:
- Graduate of an accredited Physical Therapy program.
- Current state licensure as a Physical Therapist.
- Previous experience in skilled nursing, long-term care, or geriatric rehabilitation preferred.
- Strong knowledge of Medicare Part A, Part B, and managed care therapy guidelines.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Strong clinical assessment and documentation skills.
