Landmark Home Health is a non-profit organization dedicated to delivering high-quality home health services to patients in the comfort of their own homes. We are hiring an Admissions RN in the East Pittsburgh areas. As an Admissions RN at Landmark Home you will be responsible for assessing and admitting patients to our program- as well as collaborating with physicians and healthcare professionals to ensure the smooth transition of patients into our care. Strong clinical skills, computer skills, and of course Home Health Care experience is not required but a PLUS!
We support the Public Service Loan Forgiveness (PSLF). The PSLF program helps individuals in public service roles alleviate the burden of student loans.

RESPONSIBILITIES:
- Initial evaluation and assessment of patient needs.
- Establishes a nursing care plan, implements this plan, and evaluates patients' progress.
- Sets goals with the patient, patient's family, and physician.
- Maintains a safe environment for patients.
- Completes verbal orders and maintains an up-to-date medical record.
- Provides services requiring substantial specialized nursing skills.
- Communicates, on a regular basis, with other disciplines involved in the care of the patient.
- Promotes optimum health levels for patients.
- Attends regularly scheduled staff meetings.
QUALIFICATIONS:
- Current Pennsylvania Nursing License is required
- Must have two (2) years of current nursing experience
- Must have one (1) year of home health experience
- Valid current driver's license and auto insurance coverage is required
Job Type: Full-time
Pay: $73,000.00 - $76,000.00 per year
