Responsibilities

Conducts initial and ongoing comprehensive assessments of the client's needs, including OASIS

Performs starts-of-care, resumptions-of-care, re-certifications, and discharges

Provides nursing care following established nursing service objectives and standards

Responds to emergency situations

Ensures that all safety and infection control practices are followed

Observes and reports any changes in the client’s mental, physical, or emotional condition or home situation to immediate supervisor

Ensures that required documentation is complete in a timely manner and is in compliance with regulations and standards
Qualifications

Experience in home healthcare required
