SUMMARY: Under the direction of a registered nurse, physician, physician’s assistant and FNP, assists in providing direct nursing care for patients, and in doing so, ensures all care is age-specific to include all psycho-social issues associated with patients in the various age groups. Participate in patients and family education.
FUNCTIONS AND RESPONSIBILITIES:
- May be assigned to do triage at the triage room. During this function the LVN will be under MD supervision.
- Assesses patient condition and determines nursing needs. Identifies goals and provides nursing actions including physical, psychological, emotional, social, and rehabilitative discharge planning to meet goals.
- Collaborates with patient, families, and health care team members in development, implementation, and evaluation of patient’s plan of care. Integrates cultural and age differences when developing, implementing, and evaluating patient’s plan of care.
- Provides necessary patient education for patients and family members regarding nursing diagnosis and self-care needs.
- Effectively provides nursing treatment to patients and on emergency situation, including obstetrical emergencies. Follows nursing standards, policies, and procedures when delivering nursing treatment to patient. Nursing treatment will include, but is not limited to safe handling of medications, immunization. IV therapy (if IV therapy certified), medication administration.
- Participates in orientation of new personnel. Serves as a positive role model to new employees, encouraging a high level of professional conduct and positive work attitude.
- Utilizes quality improvement systems for constructive changes. Participates in identification, implementation, and evaluation of Kima:w medical center goals.
- Participates in economic utilization of supplies and assists in maintaining infection control policies while creating a safe working environment.
- Maintains strict confidentiality with regards to patient information on EHR.
- Utilizes health care team resources to maximize quality patient care.
- Adheres to all policies and procedures of K’ima:w Medical Center.
- Participation in the CQI process to ensure JC Accreditation is maintained and on-going improvements in patient care outcomes continues.
- Participate in clinic pharmacy procedures to include appropriate medication requisitions, stock maintenance, and removal of outdated or expiring medications. Employee participates in patient case management.
- Participate in IPC team, cooperation and good team leader.
- Other duties as assigned.

2. Age specific care are defined as newborn, infants, toddlers, preschool children, school age children, adolescents, young adults, middle-age adults, and late-stage older adults.
3. QUALITY IMPROVEMENT RESPONSIBILITIES: Responsible for assisting with planning, delivery, and maintenance of high quality health care. Serve on various performance improvement committees and become directly involved in the generation or modification of policies and procedures that allow KMC to continue to improve on health care services.

JOB-SPECIFIC SKILLS AND ABILITIES:
· Demonstrated ability to perform processes in the delivery of care to specific age groups
· Skills relative to efficient emergency procedures to include: starting IV line (if IV therapy certified), EKG, cardiac monitor, and other equipment as required.
· Skill in operation of computer, calculators, and other office equipment
· Knowledge of charting, medical abbreviations and terminology, appropriate sentence structure, spelling, and punctuation.
· Demonstrated ability to obtain and interpret patient vital signs.
· Knowledge of Hoopa Community, cultural awareness and sensitivity
· Demonstrated ability to perform administrative and managerial functions, evaluate, modify, or develop new methods or revise standard methods based on the findings of investigations of similar duties
· Demonstrate the knowledge and skills necessary to provide care appropriate to any age-related needs of the patients served. The individual demonstrates knowledge of the principles of growth and development appropriate to the patient population served. He/she must be able to assess and interpret data relative to the patient’s status in order to properly identify each patient’s needs
· Demonstrates ability to prioritize responsibilities. Excellent time management skills.
· Demonstrates knowledge of maintaining and managing patient files on EHR and RPMS.
EDUCATION AND/OR EXPERIENCE: Nursing degree from an accredited school of nursing
CERTIFICATIONS, LICENSES, REGISTRATION:
- Valid California L.V.N. Licensure
- Current BLS certification
- Current ACLS certification
- Current PALS certification
ANNUAL TRAINING REQUIREMENTS: At a minimum, employee is required to be certified for the following training; KMC Annual Orientation (to include infection control, Safety and Human Resource Policy and Procedures), completion of Health Stream courses and Security Awareness Training. Both are mandatory annually.
LANGUAGE SKILLS: Ability to read and analyze professional journals, technical procedures, or government regulations. Ability to write reports and assist on procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
MATHEMATICAL SKILLS: Ability to work with mathematical concepts such as probability and statistical inference, and fundamentals of basic mathematical applications (addition, subtraction, multiplication, and division) is mandatory. Potential employee must have the ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations. Also required of employees is the ability to calculate figures in order to accurately and quickly make appropriate drug and or medication (application of 5 rights for medication administration) and fluid administration rates. Ability to convert metric equivalents as well as the ability to read and understand dosages is mandatory for all pharmaceutical functions.
RESONING ABILITY: Ability to solve practical problems and deal with a variety of concrete variables in situations where limited standardization exists is imperative. Ability to interpret a variety of instruction furnished in written, oral, diagram, or schedule form.

CONDITIONS OF EMPLOYMENT: Employee is subject to baseline and random drug testing per the Hoopa Tribal Drug & Alcohol Fit for Duty Policy. Employee will serve a 90-day introductory period. Employee is subject to introductory and annual performance evaluations. Preference will be given to qualified Indian applicants pursuant to the Tribe’s TERO Ordinance.
