Case Manager (Full-Time Employee)
Roles and Responsibilities: The Case Manager will provide direct services to program participants and ensure individualized, client-centered support. Responsibilities include: conducting intake assessments; developing individualized service and employment plans; coordinating workforce training enrollment and credential pathways; providing ongoing case management and stabilization services; providing referrals for medical, dental & mental health services; assisting with transportation and child care coordination; facilitating referrals legal partners; coordinating housing placement; tracking participant progress and documenting services provided; supporting participants in job search, resume preparation, and interview readiness; monitoring participant outcomes and reporting grant data.
Required Experience and Qualifications: Bachelor’s degree in social work, human services, criminal justice, or related field preferred; equivalent experience considered; Minimum of 2 years of case management or direct service experience; Experience working with justice-involved individuals, low-income populations, or individuals experiencing housing instability preferred; Strong organizational, documentation, and client engagement skills; Ability to maintain confidential records and comply with reporting requirements.
