Wage: $25.13/hr + DOE + DOQ
Schedule: Monday- Friday, 8:00am-4:30pm

Position Summary:
This position works with participants and is responsible for assisting people to move from unsheltered living situations to emergency shelters and stable housing. The young adult case manager will regularly visit known populated camps to build relationships with those who are unsheltered, as well as provide on-site support to participants in the YHEP program. The young adult case manager collaborates with other community partners and participates in joint outreach efforts to assist people in completing housing assessments, sign up for Oregon Health Plan (OHP), acquire documentation needed to obtain housing, and help those who are unsheltered get connected to local resources. The young adult Case Manager’s primary focus is to provide case management for young adults (aged 18-24), and may support individuals through outreach in one or more of the following areas: case management and outreach for singles, couples, and families, and/or community health work and health and housing integration.
Experience working with individuals experiencing homelessness, chronic illness, behavioral health challenges, or substance use disorders preferred.
The role requires initiative, reliability, and regular communication with supervisors regarding project progress, challenges, and development needs.
Data, Documentation, and Compliance.
Carry out medication pickup and delivery for participants as needed, ensuring secure handling, accurate receipts and forms, and compliance with C@P established process.
Build relationships and trust with unsheltered individuals and YHEP program participants.
Support participants to take the next step in stability through accessing resources, including Oregon Health Plan, domestic violence intervention, drug and alcohol treatment, behavioral and mental health services, and others.
Assist participants with achieving their goals, communicating progress with program manager and peer specialist, as well as assisting with any barriers while maintaining the dignity and choice of the participant.
Educate individuals on life skills, such as appointment management, nutrition, hygiene, and navigating systems independently.
Creating access to shelter and housing opportunities.
Cultivating the mission and values of C@P by developing and working with community partners and Church at the Park programs.
Participates in collaborative outreach opportunities alongside partnering organizations, such as Salem Housing Authority, HIV Alliance, Easter Seals, ARCHES, Northwest Human Services, etc.
Collaborate with all team members in a respectful and collaborative manner, following C@P’s values of seeing with compassion, building relationship, practicing empathetic listening and pursuing peacemaking, while utilizing the outward mindset and Non-violent communication principles.
