Position Summary
We are seeking a compassionate and motivated Bilingual Social Worker to join our interdisciplinary team. This is an excellent opportunity for both experienced candidates and recent master's graduates who are passionate about serving older adults and diverse communities. Preference will be given to candidates who are fluent in Chinese (Cantonese and/or Mandarin) or Vietnamese.

Qualifications
Education
- Bachelor's degree in Social Work or Sociology with at least 2 years of relevant social work experience, OR
- Master's degree in Social Work, Sociology, Gerontology, or Psychology.
- Recent graduates with a master's degree in one of the above fields are encouraged to apply if they have internship, practicum, volunteer, or other relevant experience in social services.
Preferred Qualifications
- Bilingual in Chinese (Cantonese and/or Mandarin), Vietnamese and English.
- Strong interpersonal, communication, and organizational skills.
- Ability to work collaboratively in a multidisciplinary team.
- Knowledge of community resources, Medicare, Medi-Cal/Medicaid, Social Security, and aging services is a plus.
Essential Duties and Responsibilities
- Conduct comprehensive biopsychosocial assessments for program participants.
- Develop individualized care and service plans based on participants' needs.
- Complete timely monthly and quarterly assessment documentation.
- Assist participants with questions and applications related to:
- Medical services
- Medicare
- Social Security
- Community resources and benefits
- Coordinate with healthcare providers, family members, and community agencies to ensure continuity of care.
- Provide referrals, advocacy, and supportive counseling as appropriate.
- Work collaboratively with a team of Social Workers, Social Work Assistants, and other interdisciplinary staff.
- Maintain accurate and confidential participant records in accordance with agency policies and regulatory requirements.

Pay: $40,000.00 - $42,000.00 per year
