What You'll Do
- Deliver skilled nursing care in patients' homes
- Administer medications and treatments as prescribed
- Monitor patient conditions and report significant changes
- Assist with skilled nursing procedures within the LPN scope of practice
- Maintain accurate and timely clinical documentation
- Educate patients and caregivers on the plan of care
- Collaborate with physicians, families, and the BrightStar Care clinical team
- Follow infection prevention and patient safety standards
What Makes You a Great Fit
Required Qualifications
- Active New York State Licensed Practical Nurse (LPN) License
- Minimum of one year of nursing experience
- Strong communication and clinical skills
- Reliable attendance and professionalism
- Ability to work independently in a home care environment
Preferred Experience
- Home care nursing
- Pediatric, adult, or geriatric care
- Experience providing one-on-one patient care
- Skilled nursing experience
