Job Summary
We are seeking a dedicated and skilled Provider—either an Advanced Practice Provider (APP) or Physician—to join our team in Creston for four 10 hour days/week. This role involves delivering comprehensive primary care services in a collaborative environment, utilizing advanced clinical skills to improve patient outcomes. The successful candidate will be instrumental in providing high-quality healthcare across diverse patient populations, including pediatric, adult, and geriatric patients, while adhering to best practices and regulatory standards.
Responsibilities
- Provide primary health care services to children and adults, including management of common acute
- and chronic illnesses, episodic history and physical exams, and minor acute conditions;
- Perform ongoing assessment of chronic medical conditions, and identifies patients at risk for specific
- health problems;
- Perform common office laboratory tests (U/A, wet mount, rapid strep screen, etc.);
- Formulate diagnosis based on interpretation of history, physical exam, and diagnostic tests, and
- develop health care plan including treatment, health maintenance education, and follow-up;
- Maintain documentation of evaluation and care provided in a complete and timely manner;
- Share in after-hours telephone triage of emergency calls;
- Offer medication-assisted treatment (MAT) for individuals with substance use disorders, including
- referral to counseling and support services to promote recovery.
- Review, understand, and follow CHC’s Scope of Clinical Services document in its current revision;
- Support and assist as requested in ongoing training of clinic volunteers, and pre-professional and
- professional students in the clinic setting;
- Identify staff and volunteer training and professional development needs;
- Participate in ongoing evaluation of existing clinical programs, and in discussion, development, and
- implementation of new programs;
- Attend staff meetings, training, and clinic community activities as required; and
- Performs other duties as assigned
If Physician:
- Candidates must be a board certified or board eligible Family Practice or Internal Medicine physician licensed to practice in Michigan;
- Must have clinical experience or equivalent combination of education and experience to successfully perform the essential duties.
- Comfortable with treating all age groups is preferred but not required
If Nurse Practitioner:
- Master’s or Doctorate’s degree from accredited Nursing Program
- Active RN and APNP license in State of Michigan AND
- Active board certification as a NP
If Physician Assistant:
- Master’s degree from accredited PA Program
- Active PA license in State of Michigan AND
- Active PA board certification
All Physician, Nurse Practitioner and Physician Assistant applicants must have:
- Active DEA license or ability to apply for such license prior to or upon hire
- CPR certification and advanced life support certifications if relevant to practice area
