Job Summary
We are seeking a Discharge Planning Assistant to help the Discharge Coordinator with gathering information Residents will need for smooth discharge. As with all Baywind emplloyees, you are to work to make sure the residents' needs are always met while exhibiting exemplary professionalism. The Discharge Planning Assistant will also be fully crossed trained to facilitate the Discharge Coordinator's responsibilities in his/her absence.
Duties
Update Calendar daily and PRN
Send insurance updates, per the Discharge Calendar
Send clinical information to chosen Home Health Company and confirm acceptance
Send order to DME company and confirm delivery date
Deliver NCMNC, when requested
Communicate all updates/work completed to DC Coordinator at the end of each day
Fill in when DC Coordinator is out of the office
Experience
- Previous experience in home healthcare is required.
- Bilingual skills are a plus to enhance communication with diverse clients.
- Familiarity with medical terminology
- Strong organizational skills and experience in office management or clerical duties are advantageous, along with strong computer literacy and excellent communication skills
- A patient service-oriented mindset with the ability to handle sensitive situations with empathy and professionalism.
- Ability to work as a team member, when necessary (salary based upon experience)
Pay: $14.00 - $17.00 per hour
