Currently seeking a Full-Time, RN Case Manager for home care patients.
Weeknight on-call rotation, with some weekend call required.
IN and IL licensure required. Hospice, home care, or oncology experience preferred.
PRN openings also available for home care patients and at our in-patient facility, the William J Riley Memorial Residence.
