Duties
- Provide personal care assistance including bathing, grooming, dressing, and mobility support to patients with diverse needs.
- Assist with Activities of Daily Living (ADLs) such as toileting, feeding, and medication reminders while maintaining patient dignity and comfort.
- Monitor and record vital signs like blood pressure, pulse, temperature, and respiration; report any abnormalities promptly.
- Support patients with mobility transfers using proper patient transfer methods and equipment such as Hoyer lifts or gait belts.
- Administer medications following prescribed instructions while adhering to home healthcare regulations and maintaining accurate medical documentation.
- Observe and document patient conditions meticulously using EMR (Electronic Medical Records) systems; communicate updates effectively to healthcare teams.
- Assist with light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe living environment for clients.
- Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care techniques and behavioral management strategies.
- Support hospice or end-of-life care patients with compassion, respecting their comfort and dignity at all times.
- Maintain patient confidentiality in compliance with HIPAA regulations and uphold the highest standards of patient care and safety.
Experience
- Prior experience working as a Home Health Aide or in assisted living facilities is highly preferred.
- Demonstrated knowledge of medical terminology, medication administration, catheter care, and vital signs monitoring.
- Experience working with geriatric patients or individuals with developmental disabilities is advantageous.
- Familiarity with EMR/EHR systems for medical documentation and record keeping.
- Certification in First Aid, Basic Life Support (BLS), CPR, and other relevant training is required or highly desirable.
- Proven ability to perform heavy lifting safely when assisting patients; experience using patient transfer equipment like Hoyer lifts is a plus.
- Strong communication skills for effective client interaction and collaboration with healthcare teams.
- Knowledge of home health care regulations and best practices for providing compassionate client-centered care.

Pay: $23.21 - $27.95 per hour
