Position Overview (or Job Description)
We are seeking an experienced Nurse Practitioner (minimum 3 years in Primary Care) who shares our mission to improve patient health and quality of life through technology-driven, patient-centered care.
Responsibilities
- Provide comprehensive primary care services for patients of all ages, including detailed history-taking, physical exams, and accurate diagnoses
- Develop personalized treatment plans, including preventive care and chronic disease management
- Collaborate closely with physicians and other healthcare professionals to deliver integrated care
- Prescribe medications, order and interpret diagnostic tests
- Maintain accurate, up-to-date electronic medical records (EMR)
- Communicate with patients with professionalism, empathy, and clarity
Qualifications
- Active NY Nurse Practitioner license
- Certified as Family NP or Adult-Gerontology Primary Care NP
- Minimum 3 years of experience in Primary Care or Family Medicine
- Strong skills in physical examinations, general illness diagnosis, and chronic disease management
- Patient-centered mindset and excellent communication skills
- Proficient in EMR usage and basic computer skills
- Commitment to continuous learning and staying updated with the latest clinical guidelines
Compensation & Benefits
- Salary: $140k+ (commensurate with experience)
