A growing long-term care organization is seeking an MDS Coordinator to oversee the Resident Assessment Instrument (RAI) process and ensure accurate, timely completion of MDS assessments. This role is ideal for an experienced RN or LPN with a strong understanding of long-term care regulations, reimbursement, and interdisciplinary care planning.
The MDS Coordinator will work closely with nursing leadership and the interdisciplinary team to support quality outcomes, regulatory compliance, and accurate reimbursement.
- Complete and coordinate MDS assessments in accordance with federal and state regulations
- Develop and update resident care plans with the interdisciplinary team
- Ensure timely and accurate documentation to support reimbursement
- Monitor compliance with MDS scheduling and submission requirements
- Educate staff on documentation and regulatory standards
- Collaborate with clinical leadership to support quality improvement initiatives
- Active RN license preferred; LPN candidates will also be considered
- Previous MDS Coordinator experience preferred
- Long-term care or skilled nursing experience required
- Experience with MatrixCare EMR strongly preferred
- Strong organizational, communication, and documentation skills
- Knowledge of Medicare, Medicaid, and MDS reimbursement processes
- RN or LPN candidates encouraged to apply
