Job Overview
Maple Lawn is looking for an LPN to help our team out on the 11P - 7A nursing staff. We are looking for the right candidate, who wants to work in environment where they are welcomed, valued, and appreciated.
Responsibilities
- Provide direct patient care, including administering medications and treatments as prescribed.
- Conduct patient assessments and monitor vital signs to ensure optimal health outcomes.
- Assist with daily living activities for patients, including personal hygiene and mobility support.
- Document patient care activities accurately in electronic health records (EHR) systems.
- Collaborate with registered nurses and physicians to develop and implement individualized care plans.
- Educate patients and their families about health management and disease prevention.
- Maintain a clean and safe environment for patients, adhering to infection control protocols.
- Participate in discharge planning and coordinate follow-up care as needed.
Qualifications
- Valid LPN license in the state of practice.
- Experience in critical care, long-term care, or acute care settings is preferred.
- Familiarity with medical terminology, ICD coding (ICD-9 & ICD-10), and EMR systems such as Epic or Cerner.
- Knowledge of patient assessment techniques and basic life support (BLS) certification is required.
- Experience working with diverse populations, including those with developmental disabilities, dementia, or Alzheimer's care.
- Strong communication skills to effectively interact with patients, families, and healthcare teams.
- Ability to perform procedures such as IV insertion, phlebotomy, catheterization, and medication administration.
- Understanding of geriatric care principles and familiarity with hospice or palliative care practices is a plus.
