LPN MDS Coordinator position available at Brooke Haven Healthcare. Candidates require an active LPN license. Long Term Care Experience preferred. Duties include: responsible to assign ICD-10 CM diagnosis codes to the resident’s medical conditions. Accountable for case management of Medicare Part A and insurance case management. MDS completion for all residents in the facility.
