Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using equipment such as Hoyer lifts or wheelchairs.
- Monitor and observe patients’ health status by taking vital signs like blood pressure, pulse, temperature, and respiration; report any changes to healthcare professionals promptly.
- Support medication administration following prescribed protocols while maintaining accurate medical documentation in Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems.
- Provide specialized care for individuals with dementia or Alzheimer’s disease, including memory care techniques and managing behavioral challenges compassionately.
- Assist with medical procedures such as catheter care, wound dressing changes, and hospice support to ensure comfort and safety.
- Prepare nutritious meals tailored to dietary needs and assist with meal planning and cooking; perform light housekeeping tasks including cleaning and laundry to maintain a safe living environment.
- Document all patient interactions accurately in compliance with HIPAA regulations, ensuring confidentiality of medical records and adherence to privacy standards.
Qualifications
- Proven experience in assisted living, nursing homes, or home health environments; familiarity with long-term care practices preferred.
- Strong knowledge of medical terminology, medication administration protocols, and patient care standards.
- Certified in CPR (Cardiopulmonary Resuscitation) and trained in hospice or dementia-specific care techniques.
- Experience working with individuals with developmental disabilities or disabilities requiring specialized support.
- Ability to operate EMR/EHR systems efficiently for documentation purposes; familiarity with home & community care practices is advantageous.
- Excellent patient observation skills coupled with compassionate caregiving abilities; capable of working independently while following healthcare plans diligently.
- Physical stamina to assist with lifting, transferring patients using Hoyer lifts or other mobility aids; capable of performing cleaning tasks and meal preparation safely.
