Dallas Nephrology Associates (DNA) is a large independent single specialty practice with numerous offices located throughout the Dallas-Fort worth Metroplex. Dallas Nephrology Associates Chronic Kidney Disease Education team has an immediate opening for an LVN or CMA to support and promote CKD education at Office Practice clinics, to include teaching CKD education classes across multiple Office Practice locations.
- Promotion of CKD Education: Promote all CKD education programs, and increase participation among patients. CKD Stage 4 Workshop Coordination
- Oversight: Coordinate CKD Stage 4 telehealth workshops, collaborate with OP staff, educators, and clinicians to ensure smooth program delivery, and provide on-site or remote support to workshop educators.
- Educational Material: Assist in creating, translating, and proofreading. Ensure accuracy and cultural relevance.
- Scheduling, Data & Reporting: Monitor CKD scheduling and follow-up, enter and track program data, and identify barriers for patients.
- Additional Duties: Provide coverage for classes and coordinator duties, support program development and quality improvement, and additional assignments as needed. Ability to provide culturally competent support. This position works closely with DNA staff and physicians to ensure every patient receives timely, culturally appropriate CKD education.
Travel to the Corporate office and multiple OP locations is required.
Travel Requirements: Travel is an important part of this role, with approximately 80% travel expected. Travel will include visits to any Dallas Nephrology Office Practice inside the DFW metroplex. We’re looking for someone who enjoys working across different locations and is comfortable with a flexible travel schedule.
Qualifications
- LVN, CMA, or similar clinical background required
- Nephrology or chronic disease experience preferred.
- Bilingual (English/Spanish) is highly preferred
- High School Diploma required
- Active & Valid Driver's License required
- Reliable transportation required
- Strong communication and teaching skills.
- EMR and basic data-tracking skills
