Encompass Community Supports, formerly known as Rappahannock Rapidan Community Services (RRCS), is hiring a Licensed Practical Nurse (LPN). At Encompass Community Supports, our full-time employees work 37.5 hours weekly.
Our Licensed Practical Nurse (LPN), provides skilled nursing services, monitoring of clients’ health status, monitor direct support staff’s follow through of physician’s orders, medical and medication administration protocols, work is performed under regular supervision, physician’s orders and some tasks delegated by RN.
ESSENTIAL FUNCTIONS:

Consults with physicians, RN, and referral sources about medication and health treatment plan for clients’ ongoing needs.

Maintains current and valid physician script ordering skilled nursing services.

Submits and obtains authorizations for skill nursing services, with RN approval.

Works with interdisciplinary teams and medical community.

Quality review of documentation and staff follow through related to (but not limited to): client medical appointments, physician’s orders, laboratory, and imaging orders.

Monitor compliance with medication administration protocol.

Verify compliance with medication storage, EMAR accuracy.

Verify staff’s completion of medication counts and record keeping of vital signs, medical logs.

Track annual physicals, preventative testing, dental and specialists’ appointment.

Aid in ordering durable medical equipment and adaptive equipment and setting up staff training.

Completes documentation including but not limited to: Plan for Supports, CMS 485 (with RN approval), quarterly reviews, and service notes.

Complies with agency and regulatory requirements.

Analyzes significant events and medication errors to ensure proper response and risk mitigation.
OTHER FUNCTIONS:

Participates in staffing and other meetings.

Completes Human Resources Training forms.

Other duties as assigned.
KNOWLEDGE, SKILLS AND ABILITIES:
Thorough knowledge of the principles, standards, practices and techniques of professional nursing; thorough knowledge of the dangers inherent in and the precautions to be taken in the use of narcotics and other medications; thorough knowledge of the treatment, diagnostic, immunization and related medical services for clients; thorough knowledge of the rules, regulations, procedures and policies pertaining to nursing services of the agency; ability to accurately maintain patient medical records and prepare reports; ability to establish and maintain effective working relationships with clients, RN, physicians and associates; knowledge of MS Office, to include Word and Excel.
COMPETENCIES:

EDUCATION AND EXPERIENCE:
Must be a Licensed Practical Nurse allowed to practice in the Commonwealth of Virginia. At least 1 year of experience in nursing, preferably in a long-term care setting and / or with people who have a developmental disability.
PHYSICAL REQUIREMENTS:
This work requires light physical effort working almost exclusively with lightweight materials (up to 25 pounds). The working conditions are good; may involve occasional exposure to some disagreeable elements; accidents are improbable other than minor injuries; health hazards are negligible; some mental effort and stress may be involved resulting in inconvenience and frustration. The worker may be exposed to bloodborne pathogens and may be required to wear specialized personal protective equipment.
SPECIAL REQUIREMENTS:

Attends training & maintains certifications as required by Agency and regulatory guidelines from licensing entities.
- Comply with Board of Nursing requirements to maintain licensure as a Licensed Practical Nurse.
