Deliver personalized, high-quality therapy services to residents in our skilled nursing facility
Evaluate, develop, and implement individualized treatment plans
Provide ongoing physical or occupational therapy services and reassessments
Document therapy services accurately and in compliance with all applicable regulations
Collaborate with the interdisciplinary team to ensure the best possible outcomes for residents
Promote independence, mobility, and quality of life for every resident you serve
Follow all facility policies, procedures, and professional standards
Current Physical Therapist (PT) or Occupational Therapist (OT) license in Colorado or eligibility through compact licensure
Passion for improving residents’ quality of life through exceptional therapy services
Strong communication and documentation skills
Skilled nursing or long-term care experience is a plus but not required
Collaborative, compassionate, and committed to delivering excellent care
