Overview
- Position :LPN LTC
- Location: Baltimore, MD
- Contract Length: 12 weeks
- Shift: Nights, 3x12s — 7:00 PM–7:30 AM
Responsibilities
- Conduct thorough patient assessments, including vital signs and physical examinations, to inform individualized care plans.
- Administer medications and injections accurately following prescribed protocols, including IV infusions when applicable.
- Monitor residents for signs of acute pain, infection, or other health changes; respond promptly with appropriate interventions.
- Assist with activities of daily living such as tube feeding, catheter care, and mobility assistance using equipment like Hoyer lifts.
- Document all patient interactions and treatments meticulously within electronic health record (EHR) systems while maintaining strict compliance with HIPAA and confidentiality policies.
- Collaborate with interdisciplinary teams to coordinate discharge planning, case management, and ongoing care strategies.
- Provide specialized dementia and memory care support, ensuring a safe environment for residents with cognitive impairments.
- Support infection control practices and participate in quality improvement initiatives aligned with long-term care regulations.
- Educate residents and their families about health conditions, medication regimens, and wellness strategies.
Requirements
- Active Maryland or Compact LVN/LPN license required.
- BLS required.
- Minimum 1year LVN/LPN experience.
