Home Health Occupational Therapist (OT)
Compensation: Guaranteed base Salary, Per point incentive pay in addition to the base salary, Per point value between $60 and $68, Earning Potential of $3,748 - $4,076+ bi-weekly, Payroll is processed bi-weekly (26 pay periods), Mileage reimbursed at $0.67/mile, and a Sign-on bonus.
Shift Type: Full-time, Monday - Friday, 8 AM to 4 PM (flexible), In the field/On the road at 15-30% of the average 40 hour work week.

Summary
We are looking for a compassionate Home Health Occupational Therapist (OT) to work one-on-one with our patients in their home environment to establish and oversee patients' occupational care needs. The Home Health Occupational Therapist (OT) is responsible for evaluating/assessing the needs of our patients and formulating treatment plans in the home care environment. An Occupational Therapist (OT) provides therapy services, including the creation and administration of care plans. A Home Health Occupational Therapist (OT) must work in a collaborative fashion across the internal interdisciplinary care team, physicians, and other stakeholders in our patient’s care.
Responsibilities
- Establish patient goals and help patients meet their unique goals by providing quality care, including assessing and interpreting evaluations and test results.
- Determine occupational therapy treatment plans in consultation with the care team and physicians.
- Administer occupational therapy treatments as ordered by a physician and direct treatment given by assistants.
- Evaluate effects of occupational therapy treatments; recommend adjustments and modifications.
- Maintain equipment and work area in a safe and clean condition.
- Complete discharge planning by consulting with the care team and physicians and assure any appropriate continuation of the therapeutic plan.
- Participate in and contribute to case conferences.
- Document patient care services by charting in patient and department records.
Qualifications
- Minimum of 2 years of experience as an Occupational Therapist (OT) with a preference for 2+ years in an acute or a post-acute care setting.
- Active state licensure as an Occupational Therapist (OT) is required.
- Proof of current CPR certification and valid driver’s license.
