Provide oversight of clients from program admission in collaboration with the parents; develop individualized service plans based on assessment reports provided. Assess needs and services that will assist in the delivery of service and advocacy. Manage community and family crises and evaluate parent/child/family progress and participates in interdisciplinary team meetings.
Qualifications: Bachelor degree in Social Work, Counseling Psychology or related field. Experience with Wraparound services and/or SED youth in community-based settings and/or MHRS experience with the mental health, child welfare and/or juvenile probation systems. Must be detailed oriented. Bilingual English/Spanish required. Able to work evening and/or weekend hours as needed. Available to be on-call 24/7. Must have reliable transportation, valid CA Driver License, proof of auto insurance, and an acceptable driving record.
