The Montana Department of Public Health and Human Services (DPHHS) has a career opportunity within the Senior Long Term Care Division.
This position is responsible for researching, developing, and implementing a complex care position with a focus on individuals who are living with brain injury, Alzheimer’s disease and related dementias (ADRD). The position will serve as a lead case manager and will report to the Senior Long Term Care Division.
The position will be responsible for designing, developing, and implementing a complex care management initiative and related ADRD policy frameworks, with support from Department leadership. The member population will include significantly complex medical conditions, neurocognitive and mental health co-morbidities. This position will be responsible for managing multiple highly complex cases simultaneously while providing oversight for placement stability.

Qualifications for this Career Opportunity
Knowledge of Brain Injury, Alzheimer's disease and related dementias, person-centered care principles, and care models
Knowledge of program development, policy drafting and evaluation.
Knowledge of regulatory requirements for Category D assisted living facilities.
Meet minimum qualifications:
Bachelor’s degree in health care, human services, business, or public administration.
Four years’ experience in case management, including complex cases.
Other combinations of education and experience may be considered on a case-by-case basis.

This position may be eligible for partial telework within the state of Montana. Specific conditions will be provided by the supervisor and must adhere to state policy.
This position may be eligible for a statutory $1.00 an hour base pay increase effective July 1, 2026.
The primary location for this position is open for consideration contingent on available workspace. Specific conditions will be provided by the supervisor and must adhere to state and DPHHS policies.
