Pay: $20.00–$25.00 per hour
About Us
Seva Senior Center is an Adult Day Health Care (ADHC) center dedicated to improving the physical, emotional, and social well-being of seniors through compassionate, person-centered care.
Responsibilities
- Complete psychosocial assessments and participant evaluations
- Develop and update individualized care plans
- Provide counseling and emotional support to participants and families
- Coordinate community resources and referrals
- Participate in interdisciplinary team meetings
- Maintain accurate and timely documentation
- Ensure compliance with ADHC/CBAS regulations
Qualifications
- Bachelor's or Master's degree in Social Work (MSW)
- Minimum 2 Years of prior experience working as Social Worker
- Experience working with seniors or in a healthcare setting preferred
- Strong communication and documentation skills
- Bilingual (English/Hindi/Gujarati/Spanish) is a plus
Schedule
- Part-Time or Full-Time
- Monday–Friday (8AM-2PM)
- Day shift only (No evenings or weekends)
