Duties
- Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts when necessary
- Monitor and observe patients’ conditions by recording vital signs, noting changes in health status, and reporting findings accurately in electronic health record (EHR) or medical documentation systems
- Provide medication reminders and assist with medication administration following home health care regulations and medical terminology guidelines
- Support patients with specialized needs such as Alzheimer’s care, dementia care, memory care, hospice care, or end-of-life support with sensitivity and professionalism
- Perform light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe living environment for clients
- Maintain patient confidentiality in compliance with HIPAA regulations while communicating effectively with clients and healthcare teams
- Use proper patient transfer techniques and equipment such as Hoyer lifts or other mobility aids to ensure safety during patient transfers
Experience
- Prior experience working with individuals with disabilities, developmental disabilities, or geriatric patients in settings such as assisted living facilities or nursing homes is highly preferred
- Demonstrated knowledge of medical terminology, catheter care, vital signs monitoring, and basic life support (BLS) or CPR certification required
- Experience working within home health agencies or providing home & community care assistance is advantageous
- Familiarity with EMR (Electronic Medical Records) systems and medical documentation procedures is beneficial
- Proven ability to deliver compassionate patient personal care assistance while maintaining professionalism and confidentiality
- Previous live-in employment experience or long-term caregiving roles are valued assets
- Ability to perform heavy lifting safely when required and work effectively within home health regulations
