Mental Health Service Coordinator

Welcome House is seeking a compassionate and motivated Mental Health Service Coordinator to provide case management, mental health support, and advocacy for individuals experiencing or at risk of homelessness. This position plays a critical role in helping participants achieve housing stability, improve mental wellness, and increase independence.
Key Responsibilities
- Manage a caseload of approximately 15–20 participants.
- Conduct mental health assessments and develop individualized service plans.
- Provide case management, crisis intervention, and ongoing participant support.
- Connect participants with mental health, healthcare, substance use, employment, and community resources.
- Conduct regular home visits and community-based meetings.
- Maintain accurate documentation and ensure compliance with HUD and agency requirements.
- Collaborate with internal teams and community partners to coordinate participant care.
Qualifications
- Bachelor's degree required; Master's degree and clinical licensure preferred.
- 3–5 years of experience in social services, behavioral health, or case management.
- Knowledge of trauma-informed care, motivational interviewing, crisis intervention, and community mental health resources.
- Strong communication, organization, documentation, and problem-solving skills.
- Ability to work independently while collaborating with multidisciplinary teams.

Pay: $21.50 - $24.00 per hour
