Job Overview
We are seeking a compassionate and highly skilled Hospice Case Manager RN to join our dedicated team. In this vital role, you will coordinate and deliver comprehensive end-of-life care, ensuring patients and their families receive the highest quality support during challenging times. Your expertise in critical care, hospital medicine, and post-acute care will drive personalized care plans, facilitate smooth transitions across care settings, and uphold the dignity and comfort of each patient. Duties
- Conduct thorough patient assessments, including vital signs, physiology evaluation, and medical history review to determine eligibility for hospice services.
- Develop and implement individualized care plans that incorporate symptom management, pain control, and psychosocial support aligned with patient wishes.
- Coordinate interdisciplinary team activities such as discharge planning, diagnostic evaluation, and utilization management to ensure seamless patient transitions across inpatient, outpatient, and post-acute settings.
- Provide expert guidance on complex cases involving infectious disease care, behavioral health issues, geriatrics, pediatrics, or trauma-related conditions like Level I or Level II trauma center experiences.
- Manage patient records meticulously in compliance with HIPAA regulations while documenting medical records, intake information, and clinical notes accurately.
- Collaborate with primary care providers and hospital staff to facilitate effective communication regarding patient care goals and treatment updates.
- Educate patients and families about disease processes, medication management, and available community resources to promote informed decision-making.
Qualifications
- Current RN licensure with active registration; experience in critical care environments such as ICU, PICU (Pediatric Intensive Care Unit), or emergency room settings is highly preferred.
- Demonstrated expertise in hospital medicine, medical-surgical nursing, or post-acute care environments; experience in managed care or utilization management is a plus.
- Strong knowledge of physiology, medical terminology, diagnostic evaluation procedures, and discharge planning processes.
- Proven ability to perform comprehensive patient assessments including vital signs and functional evaluations within acute or outpatient settings.
- Experience working with diverse populations such as geriatrics or pediatrics; familiarity with behavioral health considerations is advantageous.
- Excellent communication skills for effective collaboration with multidisciplinary teams while maintaining strict adherence to HIPAA standards.
- Ability to navigate complex cases involving trauma centers or infectious disease scenarios confidently; prior OR experience or involvement in emergency medicine is beneficial.
