Per Diem Senior Counselor wanted for our adolescent substance use IOP and adolescent Partial Hospital program.
Responsibilities:
- Provide comprehensive assessments, evaluations and diagnostic summaries for chemically dependent and dually-diagnosed patients.
- Develop patient-centered, individualized and comprehensive treatment plans. Document patient progress towards goals in treatment within OASAS and other regulatory standards.
- Facilitate and provide group therapy. Provide case management services and coordination of care with key stakeholders.
- Complete discharge summaries and appropriate aftercare planning.
Qualifications:
- Excellent written, computer and oral communication skills are necessary.
- Potential candidate will be able to work well within the framework of a high functioning team.
- Position requires Qualified Health Professional: LMHC, LCSW, LMSW, MHC-LP, MSW-LP.
