Job Summary:
The RN Case Manager is responsible for coordinating patient care across the healthcare continuum to ensure optimal clinical outcomes, efficient resource utilization, and a smooth discharge process. This role involves working closely with physicians, nurses, patients, and families to develop and implement individualized care plans.
The Case Manager plays a critical role in assessing patient needs, facilitating transitions of care, and ensuring compliance with regulatory and payer requirements in a fast-paced clinical environment.
Roles and Responsibilities:
Patient Assessment & Care Planning:
- Evaluate patients’ medical histories, current conditions, and treatment plans.
- Identify patient needs, risks, and barriers to care.
- Develop individualized care plans in collaboration with physicians, nurses, and other healthcare professionals.
Care Coordination:
- Coordinate care among multiple providers, including specialists, therapists, and social workers.
- Ensure smooth transitions between hospital, rehab, home, or other care settings.
- Arrange referrals, follow-ups, and community resources as needed.
Monitoring & Documentation:
- Track patient progress and update care plans accordingly.
- Maintain accurate and timely documentation in electronic health records (EHR).
- Ensure compliance with healthcare regulations and facility policies.
RN01
Requirements:
- Active CA RN license
- BLS(AHA)/CPR
- Minimum 2 years of Case Management experience preferred.
- Charting System: Required, Epic
Note: 2-year acute care hospital inpatient Case Manager experience performing Care Coordination and Admission Reviews, must be current experience within the last 6 months
