Registered Nurse (RN) Home Health | Full Time Facility Based

We're currently seeking a Full-Time Registered Nurse (RN) to provide skilled home health services to patients residing in assisted living, independent living, and other residential care facilities within our service area.
Position Summary
As a Facility-Based Home Health Registered Nurse, you will provide skilled nursing care to patients in residential care communities while working closely with physicians, facility staff, patients, and families to develop and implement individualized plans of care.

Responsibilities
- Perform comprehensive nursing assessments and admissions for home health patients.
- Develop, implement, and update individualized plans of care in collaboration with the physician and interdisciplinary team.
- Provide skilled nursing visits in accordance with physician orders and agency policies.
- Monitor patient progress and promptly communicate changes in condition to the physician.
- Educate patients, families, and caregivers regarding disease management, medications, treatments, and safety.
- Accurately complete OASIS assessments and electronic documentation.
- Coordinate care with therapists, social workers, physicians, facility staff, and other healthcare professionals.
- Ensure compliance with Medicare, CMS, ACHC, and agency standards.
- Participate in case conferences and interdisciplinary team meetings as needed.
QualificationsEducation
- Graduate of an accredited School of Nursing.
Licensure
- Current, active Florida Registered Nurse (RN) license.
- Current CPR/BLS certification.
Experience
- Minimum of one (1) year of RN experience required.
- Home Health experience preferred.
- OASIS documentation experience preferred.
- Strong assessment, communication, and organizational skills.
- Ability to work independently while collaborating with an interdisciplinary team.
