Duties
- Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and personal hygiene to promote dignity and comfort
- Monitor patient health by observing vital signs such as blood pressure, pulse, and temperature; report changes to healthcare professionals promptly
- Support medication administration following prescribed care plans while adhering to HIPAA regulations for patient privacy and confidentiality
- Prepare nutritious meals aligned with dietary restrictions and preferences; ensure proper food safety and sanitation standards are maintained
- Provide transportation assistance for medical appointments, errands, or social activities to foster community engagement and independence
- Utilize manual handling techniques and equipment such as Hoyer lifts for safe patient transfers; employ patient transfer methods that prioritize safety for both client and caregiver
- Maintain detailed documentation of care provided, patient observations, and any incidents; update care plans regularly in collaboration with healthcare teams
- Perform light housekeeping tasks including cleaning client living areas to ensure a safe and hygienic environment
- Engage with clients experiencing dementia or neurodevelopmental disorders through specialized memory care techniques and personalized support strategies
- Support clients with developmental or intellectual disabilities by implementing individualized support plans and facilitating social interactions
Qualifications
- Proven experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly desirable
- Strong knowledge of patient observation techniques, vital signs monitoring, medication administration, and first aid/CPR certification
- Familiarity with dementia care, Alzheimer’s care, hospice care, autism spectrum disorder (ASD), intellectual disabilities support, and neurodevelopmental disorders
- Experience operating patient transfer equipment such as Hoyer lifts; manual handling skills essential for safe patient transfers
- Ability to communicate effectively with clients and healthcare teams while maintaining confidentiality in accordance with HIPAA standards
- Demonstrated ability to perform heavy lifting safely when required; experience working with individuals with disabilities or developmental challenges is a plus
- Compassionate attitude combined with excellent organizational skills to manage multiple care tasks efficiently
- Valid driver’s license preferred for personal transportation assistance; background check required for compliance with safety standards
