Responsibilities
- Assist residents or clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts or transfer belts
- Monitor patients’ vital signs and observe their overall condition to detect changes or concerns promptly
- Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary
- Administer medications following care plans and ensure compliance with HIPAA regulations to protect patient privacy
- Maintain a clean and safe environment through regular cleaning, sanitation, and organization of living spaces
- Support patients with memory care needs, including dementia or Alzheimer’s care, utilizing specialized techniques and routines
- Document care activities accurately in accordance with healthcare standards and coordinate with healthcare professionals to update care plans
- Provide emotional support and companionship to foster a positive environment for residents or clients
- Assist with transportation or outings within the community to promote social engagement and activity participation
Skills
- Proven experience in home & community care, assisted living, nursing homes, or long-term care settings
- Knowledge of patient observation techniques and vital signs monitoring
- Familiarity with Alzheimer’s disease, dementia, autism spectrum disorders, and developmental disabilities care strategies
- Ability to perform heavy lifting safely and operate mobility aids such as Hoyer lifts or wheelchairs
- Strong understanding of medication administration protocols and HIPAA compliance
- Excellent communication skills to effectively collaborate with team members, residents, families, and healthcare providers
- CPR and First Aid certification to respond confidently during emergencies
- Experience working with individuals with disabilities or special needs in a supportive environment
- Capable of managing multiple responsibilities while maintaining attention to detail and compassion
