Title: Client Care Coordinator, LMSW & CASAC
Supervisor: Program Director LMSW
Category: Exempt
Hours: This position is for 6-hour days - Monday to Friday 30 hours per week
Job Summary:
Conduct psycho-social assessment and development of ILP to identify and address barriers related to chronic homelessness and expedite housing placement. Ability to supervise case manager and case load capacity of 1-27.5
Duties:
A) Conduct psycho-socials and ILP on a new family arrivals within 48 hours.
B) Arrange and coordinate the implementation of ILP
C) Arrange and schedule all weekly meeting with families and case managers.
D) Coordinate schedule required medical, mental health and entitlement benefits assessments.
E) Conduct monthly case conference with case managers and housing specialist.
F) Conduct crisis interventions as needed
G) Maintain accurate case records, progress notes and ILP summaries for each client
H) Organize and maintain related community resources
I) Work closely with existing offsite service providers.
J) Provide general social work to all clients
Qualifications:
Education:
LMSW, CASAC certification a plus
Experience:
Strong organizational skills, minimum of 3 years experience in working with homeless families with special needs, Bilingual a plus.
Job Type: Full-time
Pay: $68,500.00 per year
