This creates a stable, outpatient-only environment where providers collaborate closely with Primary Care Providers, care managers, and interdisciplinary teams to deliver comprehensive, whole-person care.
Our Behavioral Health Specialists focus on what matters most: delivering exceptional patient care without hospital call, inpatient responsibilities, or fragmented treatment.

Conduct comprehensive mental health assessments.

Develop individualized treatment plans.

Provide outpatient therapy for adults and/or children.

Deliver crisis intervention and behavioral stabilization when appropriate.

Maintain a structured caseload of one patient per hour to ensure high-quality care.

Partner closely with Primary Care Providers within our MindPATH model.

Collaborate with care managers and interdisciplinary teams.

Connect patients with appropriate behavioral health and community resources.

Complete accurate clinical documentation, treatment plans, and progress notes.

Monitor patient outcomes and adjust treatment plans as needed.

Prepare discharge plans and case summaries.

Master's degree in Social Work, Counseling, or a related field.

Active LPC or LMSW license.

Experience conducting assessments, treatment planning, therapy, and crisis intervention.

Three or more years of outpatient behavioral health experience.

Comfortable working in a collaborative, integrated care environment.

Passion for serving vulnerable and underserved populations.

Strong communication and interdisciplinary teamwork skills.
