Physical Therapist (Contract – Travel or Perm)

Schedule & Assignment Details:

Location: SNF facility, CA 94952
Start Date: ASAP
Duration: 13+ week contract
Hours: 36 hours/week
Pay: $95+/hour
Setting: Skilled Nursing Facility (SNF)
Employment Type: Travel or Permanent

Position Summary & Responsibilities:

Evaluate and treat SNF residents requiring physical therapy services
Develop and implement individualized treatment plans to improve mobility and function
Perform ongoing reassessments and adjust care plans as needed
Document evaluations, treatments, and progress in compliance with facility standards
Collaborate with interdisciplinary team to support optimal patient outcomes
Educate patients and caregivers on exercises, safety, and rehabilitation goals
