Serenity Hospice PA is seeking a compassionate and dedicated Hospice RN Case Manager (RNCM) to join our growing team serving patients throughout Dauphin County, PA.
This position offers the opportunity to make a meaningful impact by providing compassionate end-of-life care and support to patients and their families in the comfort of their homes.
Manage and coordinate hospice patient care plans
Perform comprehensive nursing assessments
Provide symptom management and patient education
Collaborate with physicians, caregivers, and interdisciplinary team members
Maintain accurate and timely documentation
Support patients and families emotionally throughout the hospice journey
Active Pennsylvania RN license
Hospice or home health experience preferred
Strong clinical and organizational skills
Excellent communication and compassionate patient care abilities
Valid driver’s license and reliable transportation
