What your will do:
The Patient Service Coordinator is responsible for coordinating and scheduling patient visits for field clinicians while performing administrative functions that support patient care. This role collaborates with clinicians, patients, caregivers, providers, and other Home Care Interdisciplinary Group (IDG) members to ensure effective care coordination, continuity of services, and smooth transitions within Home Care.
The Patient Service Coordinator serves as the primary resource for patient scheduling and must maintain a thorough understanding of regulatory requirements related to Home Care scheduling and visit management. Responsibilities include managing clinician schedules, participating in holiday staffing, updating the clinical on-call schedule, monitoring and processing patient information and physician orders, and completing order follow-up activities. Additional duties include managing supply requisitions and maintaining adequate inventory of clinical supplies, providing scheduling and operational updates during clinical meetings, actively participating in Case Conferences, and performing other administrative tasks as assigned. The Patient Service Coordinator works closely with the Home Care team to support efficient operations and high-quality patient care.
Required Education:
High School diploma or equivalent
Required Technical Skills:
Basic knowledge and skills to care for the needs of the population served. Professional and efficient verbal and written communication skills. Professional interpersonal skills. Ability to multi-task, basic organizational and time management skills. Basic computer skills. Knowledge of general office and administrative practices and procedures. Knowledge of how to utilize office equipment (copy machine, fax machine, etc.).
Preferred Skills:
Experience with Home Care Home Base; Experience with smart phones and smart devices
Required Licensure/ Registration/ Certification:
Must have reliable transportation
