Caring Connections is seeking a compassionate and reliable Registered Nurse (RN) to join our team. This immediate opening is available on a full-time basis, with responsibilities focused on conducting in-home assessments for clients residing throughout Westchester County, NY.
This role offers the opportunity to make a meaningful impact by ensuring high-quality care for clients in their homes. While experience is preferred, we are open to considering new RN graduates who are eager to learn and grow in a supportive environment.
Key Responsibilities:
- Conduct in-home assessments to evaluate clients' health needs and develop care plans.
- Communicate findings and collaborate with the Director of Patient Services (DPS) to ensure the best possible care.
- Maintain accurate and timely documentation of assessments.
- Build trusting relationships with clients and their families to promote well-being.
Requirements:
- Active Registered Nurse (RN) license in the state of New York.
- A personal vehicle available for commuting and travel to client homes across Westchester County.
- Strong organizational and communication skills.
- Flexibility and willingness to adapt to a dynamic work schedule.
- Commitment to providing compassionate and professional care.
Preferred Qualifications:
- Prior experience in home health care or a similar setting is a plus.
- Familiarity with conducting health assessments.
