Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. Responsibilities
- Perform comprehensive patient assessments, including vital signs, physical examinations, and medical history documentation using electronic health record (EHR) systems such as Epic or Cerner.
- Administer medications and injections accurately following prescribed protocols, including IV infusions and medication management.
- Assist with diagnostic evaluations, specimen collection, and processing to support physicians’ treatment plans.
- Manage airway care, tube feeding, catheterization, wound care, and sterile procedures in compliance with aseptic techniques.
- Monitor patients’ conditions closely, recognize signs of acute pain or deterioration, and escalate concerns promptly to the healthcare team.
- Provide patient education on health management topics such as dementia care, Alzheimer's care, pain management strategies, and long-term care regulations.
- Collaborate effectively with interdisciplinary teams in various settings including ICU, emergency room, skilled nursing facilities, hospice care, and pediatric units to ensure seamless patient care delivery.
Skills
- Proven experience working with diverse patient populations including geriatrics, pediatrics, individuals with developmental disabilities or disabilities.
- Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), and medical documentation standards.
- Skilled in using EMR/EHR systems like Epic, Cerner or Athenahealth for accurate record keeping and documentation.
- Expertise in critical care environments such as ICU or Level I Trauma Centers; experience with emergency medicine or urgent care is highly valued.
- Proficiency in IV insertion/infusion techniques including venipuncture and catheter care; familiarity with ventilator management preferred.
- Ability to perform physical examinations and diagnostic evaluations while maintaining strict adherence to HIPAA and clinical confidentiality policies.
- Excellent communication skills for effective caregiver communication and client engagement; experience working with individuals with disabilities or complex medical needs is a plus.
