MDS Coordinator (RN)

What You'll Do:
Coordinate and complete the MDS assessment process in accordance with federal and state regulations.
Ensure accurate and timely completion of MDS assessments, care area assessments, and care plans.
Collaborate with the interdisciplinary team to develop and update individualized care plans.
Monitor Medicare and Medicaid reimbursement opportunities while maintaining compliance.
Review documentation to ensure accuracy and support reimbursement.
Participate in quality assurance and regulatory compliance activities.
Educate and support nursing staff regarding documentation and MDS requirements.
