As a key member of our medical team, you will play a vital role in delivering palliative care services, collaborating with multidisciplinary teams to develop effective treatment plans tailored to each patient's needs. - Provide comprehensive palliative care assessments and recommendations

Collaborate with patient families and healthcare teams for holistic treatment

Develop individualized care plans that address physical, emotional, and spiritual needs

Conduct regular patient evaluations and document care progress

Communicate effectively with patients and their families to support decision-making

Participate in interdisciplinary team meetings and case discussions

Educate patients, families, and staff on palliative care principles

Stay current with best practices in palliative care and related fields
- MD or DO degree from an accredited institution

Experience in palliative care, geriatrics, or internal medicine is preferred

Valid medical license to practice in the state

Strong clinical and diagnostic skills

Schedule: Monday-Friday, 8a-5p
Daily Volume: 10 admissions
Patient Population: Geriatric
Duties: Managing chronic conditions, preventative care & assessments, medication management, and care coordination
Support Staff: MA and RN
EMR: Cerner
Must be board-certified
