Qualifications
- Active Registered Nurse (RN) license in the state of practice.
- At least one (1) year of professional nursing experience required.
- Strong interpersonal skills with the ability to build positive relationships with patients, families, physicians, and healthcare team members.
- Excellent assessment, communication, and documentation skills.
Position Summary
The Registered Nurse is responsible for delivering skilled nursing services to patients in their homes. This role includes patient assessment, care planning, coordination of services, clinical documentation, and ongoing communication with physicians and interdisciplinary team members to ensure quality patient outcomes.
Essential Duties and Responsibilities
The RN will perform duties including, but not limited to:
- Complete comprehensive patient assessments during initial and follow-up home visits.
- Develop, implement, and revise individualized plans of care based on physician orders and patient needs.
- Perform admissions, transfers, resumptions of care, recertifications, and discharge assessments, including all required OASIS documentation.
- Evaluate patient status and identify changes in condition, reporting significant findings to the appropriate healthcare providers.
- Monitor treatment effectiveness and patient response to interventions.
- Provide education to patients and family members regarding diagnoses, medications, treatment plans, safety measures, and disease management.
- Instruct patients and caregivers on proper techniques to support care in the home setting.
- Collaborate with physicians, therapists, case managers, and other healthcare professionals to coordinate patient care.
- Supervise and provide clinical oversight to LPNs/LVNs, HHAs, and other assigned personnel.
- Ensure all skilled nursing services are provided in accordance with agency policies and professional standards.
- Participate in interdisciplinary case conferences to promote continuity of care.
- Maintain timely, accurate, and complete clinical documentation.
- Determine when patients have achieved goals and are appropriate for discharge from skilled nursing services.
- Participate in ongoing training, continuing education, and professional development activities.
- Conduct supervisory visits as required for assigned support staff.
- Maintain compliance with all applicable federal, state, Medicare, Medicaid, and agency regulations.
