Position Summary
We are seeking a dynamic, compassionate Case Manager to join our outreach-driven team. This role is central to connecting individuals with essential services, supporting ongoing stabilization, and helping clients navigate complex systems like healthcare, housing, and benefits.
You’ll work directly with clients in the community, providing outreach, education, coordination, and follow-through support that turns connection into real outcomes.
This is a high-impact, people-facing role for someone who is energized by fieldwork, relationship-building, and helping individuals move forward step-by-step.
Key ResponsibilitiesOutreach & Client Engagement
- Conduct community-based outreach to engage individuals experiencing homelessness or instability
- Build trust with clients through consistent, compassionate contact and follow-up
- Assess client needs and connect them to appropriate resources and services
- Use motivational interviewing techniques to support engagement and positive behavior change
Case Management & Navigation
- Support clients in accessing healthcare, housing resources, recovery services, and benefits
- Help clients navigate systems such as Medicaid, Medicare, and public assistance programs
- Coordinate referrals with shelters, treatment providers, social workers, and community partners
- Provide ongoing case follow-up to ensure continuity of care and service completion
Education & Support
- Educate clients on health-related topics including nutrition, medication management, addiction recovery, and wellness strategies
- Provide clear, accessible explanations of systems, paperwork, and next steps
- Support clients in building stability through consistent coaching and encouragement
Documentation & Data Tracking
- Maintain accurate and timely case notes, outreach logs, and service documentation
- Collect and track data related to client engagement, service utilization, and outcomes
- Support reporting efforts to improve program effectiveness and impact
Community Collaboration
- Work closely with healthcare providers, housing partners, shelters, and community organizations
- Participate in coordination meetings and maintain strong interagency relationships
- Represent Momentum360 in a professional and compassionate manner during outreach and events
Public-Facing Work
- Support or participate in community education efforts and outreach events
- Assist with workshops or informational sessions as needed
- Help increase awareness of available resources and support pathways
Requirements
- Experience working with vulnerable populations (homelessness, recovery, housing instability, etc.)
- Strong communication skills and ability to build trust quickly with diverse populations
- Familiarity with or willingness to learn Medicaid, Medicare, HIPAA, and public benefits systems
- Comfort with field-based work and community outreach
- Ability to maintain accurate documentation and manage case tracking systems
- Strong interpersonal skills and ability to motivate, support, and guide individuals
- Valid driver’s license and willingness to travel throughout the community

Pay: $20.50 - $25.00 per hour
