MA - Chronic Care Manager - Rural Health Care

The Medical Assistant (MA) Chronic Care Manager (CCM) is a clinic-based position responsible for managing patients who have chronic health conditions, working towards better health outcomes for those patients, and maintaining routine contact regarding their ongoing healthcare needs. The MA CCM supports high risk and chronic conditions patients in collaboration with other team members. The MA CCM will provide services such as patient education, self-management support, coordination of care, and acting as a patient advocate. The MA CCM works as an extension of clinic providers by establishing and maintaining a more centralized coordination of care. The MA, in general, usually does not provide direct patient care, however, will assist in other clinic duties as needed, which may include delivery of direct care. The MA CCM functions under the direction of the Rural Health Clinic Manager and/or the Medical Provider(s) and exercises independent judgment in applying professional knowledge required to plan, prioritize and organize the delivery of quality patient care coordination.
This position is safety sensitive and subject to WMRMC’s Drug & Alcohol Policy.

Required Education
High School Diploma or Equivalent
Required Experience
General knowledge of medications frequently prescribed in chronic care, nursing process and it’s application, including the assessment, planning, implementation and evaluation of care
General knowledge of healthcare field
Medical terminology knowledge
EMR experience
Preferred Education and Experience
Two (2) years previous MA experience in patient care setting
Working with chronic conditions patients
Care coordination
Ability to understand and interpret statistical data related to patient care and outcomes
Continuous Quality Improvement (CQI)
Required Licenses & Certifications
Medical Assistant Certification
Basic Life Support (BLS) within 30 days of hire
