Location: Oak Village Healthcare – Oaktown, IN
Schedule: PRN
Pay Rate: $65/hr (5-10 hrs C-PRN Weekly)*
Revital Rehab Services is seeking a dedicated and licensed Physical Therapist (PT) to provide PRN therapeutic care at our partner facility, Oak Village Healthcare in Oaktown, IN. This is a rewarding opportunity to help residents improve strength, mobility, function, and overall quality of life in a supportive, collaborative, and patient-focused environment.
Conduct initial evaluations and create individualized physical therapy treatment plans
Provide evidence-based, goal-directed therapeutic interventions
Monitor patient progress, update goals, and document outcomes accurately
Collaborate closely with the interdisciplinary care team
Ensure all services meet Revital’s high standards for quality, documentation, and ethical care
Licensed Physical Therapist (PT) in the state of Indiana
Current BLS certification
Strong clinical judgment and communication skills
