Job Overview
We are seeking a dedicated and compassionate Registered Nurse - Home Health to join our team. In this role, you will provide high-quality nursing care to patients in their homes, ensuring they receive the necessary support for their health and well-being. The ideal candidate will have experience in various medical settings, including Level I and Level II Trauma Centers, and possess a strong foundation in emergency medicine, physiology, and both inpatient and outpatient care.
Responsibilities
- Conduct comprehensive patient assessments to develop individualized care plans.
- Monitor patient progress and adjust care plans as necessary.
- Provide education to patients and families regarding health management and disease prevention.
- Collaborate with interdisciplinary teams to ensure coordinated care.
- Document all patient interactions accurately in electronic health records.
- Support patients transitioning from hospital to home care settings.
Experience
- Current Registered Nurse (RN) license in good standing.
- Excellent communication skills, both verbal and written.
- Ability to work independently while managing multiple patient cases effectively.

Pay: $44.27 - $50.00 per hour
