Registered Nurse Case Manager
Duties:
- Conduct patient assessments and develop individualized care plans
- Coordinate and manage hospice patient care across many home settings
- Utilize management and organization techniques to ensure appropriate use of resources
- Collaborate with interdisciplinary teams to provide comprehensive patient care
- Monitor and evaluate patient progress and adjust care plans as needed
- Provide education and support to patients and their families
- Maintain accurate and up-to-date medical records

Skills:

- Familiarity with terminal illness environments
- Excellent communication and interpersonal skills
- Detail-oriented with strong organizational skills
As a Registered Nurse Case Manager, you will play a vital role in coordinating and managing the care of patients. Your expertise in utilization time management, physiology, and behavioral health will be essential in providing high-quality patient care and family support. Please note that this is not an entry-level position. Prior experience as a Registered Nurse is required. Hospice experience preferred.
